    Accessing NHS Records

I've just completed and emailed the forms required to get a copy of my records and received a confirmation email with some interesting wording:

    to me
    Good afternoon,
Thank you for your correspondence. We advise that before release of any medical records, they must first be assessed by a Health Professional to determine that the records for disclosure are in order, for example that there are no mixed notes and that the records released are relevant to your application.

Has anyone else gone through the process and are they actually redacting records?

Given the frequency of notes letters entered in the wrong record it would be a breach not to check first for this or redact other information about third parties (eg other patients) which shouldn't be released.

I've had some battles to get notes from other teams and trusts when i had the patient's consent, but never requested my own. For example when their own notes contradicted their decision to decline treatment.

Not redacting as far as I know - as suggested, more about being in order and relevant to you.
