I adore Grand Seiko's but I feel like new ones are a bit big for my taste. Do they produce any smaller size ones (e.g. under 40mm)? Maybe in Japan?

I think some of the simpler 3 handers are smaller, 37mm in some cases.
There are plenty under 40mm, fewer under 38mm. I really like the SBGR051 and 53, automatic, 37mm (the latest versions I think SBGR251/3). Quite a few designed around the same 39.5mm case design, including GMT and three handers. The new VFA's also 39.5mm. Plenty of quartz options too. Probably easiest to browse the GS websites, international and domestic and some of the UK dealers like Jura who list dimensions under specs.

Was looking over their site recently and agree the sports models are all massive, if not in diameter then in height. Their smarter watches do come in smaller sizes but I prefer their sports lines. All too big for my taste.

One that I really fancy is the SbGW235, a 37mm manual wind.
My former SBGX009, 36mm, quartz and very unfussy.
The automatics are 37mm, like my sbgr053, my favourite watch. Perfect size.
I think you’ll struggle if you want Spring Drive though... they all tend to be bigger.
All the quartz models are at or below 40mm, I think. The biggest are the 44GS cases (e.g. SBGV005) and these are 40mm. I'm pretty sure the whole SBGX range is 37mm or below.
I don't think the height on any of them is above 11mm so they wear extremely comfortably.
I have two 37mm Grand Seiko's from their historical collection, the automatic Sbgr095 and the quartz sbgv011. Both are stunning and wear a tad larger, more like 38 or 39mm.
I would love a Spring Drive if they were 37-39mm.

Here is a 37mm quartz, 061, on the wrist of a small woman.and on my seven and a half inch wrist.
By comparison, here's the GS 'next size up'....39mm. unless you take the now unavailable 253 manual wind, at 38mm.
In practice, the quartz 37mm series are the smallest available. They are about as small as most men would wear; in comparison the typical GS 'sports' models are about 44mm. I agree they're too big.

I remember there was a 39mm titanium spring drive available a few years ago.
I don't think there are any 39mm models currently available though.

This is a grand Seiko but in a custom credor case? Will (as claimed by seller) Seiko do that?
I recently got one for the high beat GMT's, they are 40mm and the 44GS case. I've got slim wrists and it fits me nicely. The lugs are quite short, and the dial is not huge despite it being an all dial style. The 44GS case gives it a little more bulk. The main issue fit wise is the height that makes the whole watch a bit top balanced.
